REGISTRATION INFORMATION

Delegates can register by email, mail or fax. All delegates (including invited speakers) must complete a registration form for administrative purposes.

Full Registration includes:

• attendance at all conference sessions;
• morning/afternoon teas and lunches;
• delegate kit with program and book of papers/abstracts;

Other Health Professional and Trainees: Registration at concessional rates is available to medical trainees, nursing staff, speech therapists, technologists and students. To qualify, applicants must be working as a resident, registrar, fellow, nurse, speech therapist, technologist or be a fulltime student. Support documentation confirming your current professional status must accompany the registration form.

Method of Payment
Payment must accompany the registration form and should include (as appropriate): registration fees, hotel deposit (one night’s tariff), workshop fees and social functions. Payments can be made by EFT, credit card, cheque or for international delegates, by bank draft in Australian dollars. Personal or company cheques will not be accepted from international delegates.

Acknowledgement
Registrations received prior to Friday 24 July 2009 will be acknowledged in writing. Delegates are asked to check their confirmation letter and advise the organisers immediately, if changes/amendments are required. For registrations made after Friday 24 July 2009, please telephone or email the secretariat to confirm receipt of your registration.

Registration Desk
The registration desk, located in the Southern Cross Ballroom foyer of the Fremantle Esplanade Hotel, will open each day of the conference from 8.30am - 4.00pm for the collection of delegate kits and general assistance.

Dietary Requirements
Delegates with special dietary needs must advise Promaco Conventions via the registration form or in writing to ensure they are suitably catered for.

 

CANCELLATIONS AND REFUNDS

Registration - cancellation of registration must be advised in writing. Cancellations made before Friday 10 July 2009 incur a penalty fee of A$132. Refunds of fees after this time will only be made in exceptional circumstances. Substitute delegates will be accepted and details must be advised in writing. Please note that cancellations made in the few weeks prior to the conference, will be processed after the conference concludes.

Accommodation - cancellation of accommodation must be advised in writing. Each hotel reserves the right to charge delegates for non-arrival on the dates pre-booked and/or no-shows.

Social Functions - no refunds will be given for social functions cancelled by delegates after Friday 24 July 2009 or missed during the conference.

If, for reasons beyond the control of the organisers, the conference were to be cancelled, fees minus deduction of expenses incurred would be refunded.

Please keep a copy of your registration form and note that by submitting the form, you have agreed to the terms of the cancellation policy.
